We recommend using the same documentation for your state used by FannieMae. FannieMae are the largest purchasers of conforming mortgages in the USA. By using their forms you can be pretty well assured  that your documents are legally enforceable. Also you will have a commonly understood document if you ever want to sell your mortgage.
